Scheduling and Appointment Information
  • New patients are asked to arrive 15 minutes prior to their scheduled appointment time.
  • Please bring (or take photos of) all the medications, both oral and topical, that you are taking. This includes over the counter medications and supplements. Also include dosage and frequency.
  • Every effort is made to accommodate your needs and specific concerns at each visit, however, if you have a list of multiple concerns or if you are scheduled for a full skin exam and have additional skin concerns, we may ask you to make an additional appointment so that all your questions can be addressed completely and thoroughly.
  • If you are scheduled for a full skin exam, please remove all makeup and nail polish prior to your visit.
Insurance Information
  • We accept most health insurance plans, but due to the constantly changing nature of health insurance, we ask that you please check with your health insurance carrier prior to your visit in regards to coverage and the need for a referral.
  • We are not contracted with the state HMO plans such as: Molina, CHPW, Coordinated Care, Amerigroup, and United Healthcare Community Plan.
Billing Information
  • Co-pays are due at the time of service.
  • If you do not have insurance, payment is required at the time of service.
  • Cosmetic procedures are not covered by insurance. Payment for these are required at the time of service.
  • The amount of your deductible is determined by your insurance plan; payment of your deductible may be requested at the time of your visit.
  • Please pay your bill upon receipt. A $3 re-billing fee will apply monthly after 60-days of claims processing date.
  • We accept Visa, Mastercard, Discover Card, and American Express payments online at https://payspandirect.payspanhealth.com/EDIPS, by mail or phone, as well as check (by mail or in office) and cash payments.
Cancellation Policy
  • If you need to change or cancel your appointment, please do so with at least 24 hours advance notice. All efforts will be made to try to reschedule your appointment according to your scheduling needs.
  • If you do not show up for your scheduled appointment you will be charged a $25 fee for each 15 minutes scheduled.
  • If you give less than 24 hours cancellation notice, you may be charged for the appointment.
Patients under the age of 18
  • A parent or legal guardian must accompany patients under the age of 18 (minors) at the time of their initial visit.
  • Minors can be seen unaccompanied on follow-up visits, if granted permission by their parent or legal guardian.
  • Unaccompanied minors at follow-up visits are still expected to make co-payments and update insurance information as needed.
  • Written consent must be obtained in person by a parent or guardian prior to procedures performed on minors.
Biopsy and Lab Results
  • Results of any labs, cultures, or biopsies will be communicated to you by phone call. A message regarding results will be left if you have consented for us to leave a message. Please make sure that at least one of your contact numbers has your name in the greeting as we are unable to leave messages on unidentified numbers.
Refill requests
  • When you need refills of your prescription medications, please call your pharmacy to request them. If you have no refills left on your prescription the pharmacist will contact our office to request refills. Please allow 48 hours for a response.
